Principal Care Management (PCM) Nurse

This is a remote position. Only LATAM-based candidates (bilingual in Spanish and English) Location:Latin America (Remote) Schedule:Full-time – U.S. business hours (EST) Payment:$1,200 USD/month (depending on experience) We are seeking alicensed nurseorMDto support a U.S.-based endocrinology practice through itsPrincipal Care Management (PCM) program. PCM is a care coordination program designed for patients with asingle chronic condition requiring ongoing management. In this role, you will work closely with the provider to ensure patients receive continuous support between visits, helping improve outcomes and adherence to treatment plans. This is aclinical, patient-facing role, requiring strong judgment, communication skills, and the ability to actively monitor and guide patients throughout their care journey. Key Responsibilities - Maintainongoing communication with patientsenrolled in the PCM program - Monitor patient conditions, including: - Symptoms - Medication adherence - Progress and response to treatment - Providepatient educationregarding their condition and care plan - Assist in theimplementation and follow-up of care plans - Coordinate services and ensure continuity of care - Identify changes in patient status andescalate concerns to the provider when needed - Ensure patients follow treatment recommendations and scheduled appointments - Accurately document all patient interactions and updates in the system Requirements - Licensed Nurse or MD (required) - Advanced English level (C1 or higher)– excellent spoken and written communication - Proven experience working withU.S.-based medical offices - Experience withchronic care management, patient follow-up, or care coordination - Strong clinical judgment and ability to assess patient conditions remotely - Experience providingpatient education and support - Highly organized with strong documentation skills - Comfortable working independently in a remote environment Technical Requirements - Reliable computer (8GB RAM minimum) - Stable internet connection (minimum 100 Mbps) - Quiet and professional workspace - Headset with microphone Benefits - Long-term, stable opportunity with a U.S.-based medical practice - Fully remote work environment - Opportunity to work in aclinical support role with real patient impact - Supportive team and structured workflows - 50 USD in productivity bonus (paid in local currency) - 25 USD in reliability bonus (paid in local currency) - 50 USD in consecutive reliability bonus (paid in local currency) Important - This isnot a general virtual assistant role. - Due to the clinical nature of PCM services,only licensed nurses and physicians will be considered. - Schedule:Monday through Friday, 9:00 a.m. till 7:00 p.m. Florida time (EST). - Hours:45 hours per week. - Breaks:1-hour lunch break and one 30-minute break.
